Self-Declaration form (SDF) Guide

To register for support and access potential reasonable adjustments, you will need to complete an SDF and supply supporting evidence. Please review the steps below.

Log into your e:Vision account

On the main menu, click on Disability and Inclusion

To the right, where on a PC, you will see the SDF - self declaration form tab, click on this and complete the online form. You can exit the form any time and return to the same position you left it. The form also allows you to navigate back to reenter any information you have provided. 

Within the SDF there is a document upload option. If you do not have any evidence to upload, you can continue to submit the form without evidence. Please ensure you check over your answers and press submit. Once submitted you will receive a acknowledge receipt. 

Once you have evidence to provide, you can use the Document upload task located underneath the SDF to upload this onto your record, you do not need to resubmit the SDF. 

 

 

Please see below a list of types of evidence you can provide. Please ensure that you are uploading these under the correct heading. 

Supporting evidence types: 

  • Medical evidence; this can be a doctor's letter/report or DSA medical evidence form
  • Diagnostic assessment of a specific learning difficulty (e.g. dyslexia, dyscalculia)
  • An Education, Health and Care Plan (EHCP) (or statement of Special Educational Needs)
  • A Needs Assessment Report
  • DSA entitlement letter
